If you are over 18 years old, have a mental health or neurodevelopmental condition, have a Plan-Managed or Self-Managed NDIS plan, or would like to pay privately or using a Medicare plan, and would like to work with Susan to create meaningful change, or you are a carer or care co-ordinator of such a person, please email ValuedDirectionsOT@gmail.com or complete the referral form on the REFER page. Susan currently accepts home visit referrals for NDIS participants within 15 minutes travel from the Hobart CBD, and for participants who do not have cats at home (due to allergies), or clinic referrals for those that can see her at The Hobart Health Centre, 143 Murray St, Hobart or via telehealth.
Please send an email with a brief summary of the participant (including age, diagnoses, suburb, OT hours available and reason for OT request) and Susan can answer questions or send you a link to a referral form.
